| Beginning September 1, 2007, mail the yellow research copies of all
your agencies' completed and voided PCR's to:

You can also feel free to drop them off to the office at the address
above. PCR's are still to be sent to us by the 10th of the month
following the month of the call. For example, all September PCR's
should be received by October 10th. Continue to include a summary
detailing your agency code, agency name, and contact information, the
number of new blank "NYS DOH Version 5" PCR's you have received, the
number of PCR's used by your agency, and the number of voided PCR's.
Please let me know if your agency used a form other than a NYS DOH
Version 5 Paper PCR or if electronic PCR submission to the State is
used; also where you receive your PCR's from.
